Payment Plan - 3 Monthly Payments of $298.34 for a total of $895. A $50 fee has been added to the payment plan.

Terms and Conditions

I verify that all of the information given in this registration form is true.
I understand that all fees are non-refundable. If trainee desires to change to an in-person workshop, a $300 fee will be applied. This $300 fee is due immediately to reserve the trainee’s spot in the new workshop.
I understand that I will have 365 days to complete this course. At that time, I may choose to purchase a 3-month extension for $175.
Payment plans run for 3 months. The first payment is due at registration. The remaining 2 equal payments will automatically be withdrawn on the same day of each of the following 2 months.
If a card is declined for the 2nd or 3rd payment, you will immediately lose access to your training modules, and it will not be granted again until the payment is made.
You will not receive your printed materials in the mail until all 3 payments have been made.
I will allow Birth Boot Camp to use quotes from my training evaluations (which I may fill out anonymously) for promotional purposes online and in print.
Birth Boot Camp reserves the right to refuse any registrations.
