Terms and Conditions

• I verify that all of the information given in this registration form is true.
• I understand that all fees are non-refundable. If trainee desires to change to an in-person workshop, a $300 fee will be applied. This $300 fee is due immediately to reserve the trainee’s spot in the new workshop.
• I understand that I will have 365 days to complete this course. At that time, I may choose to purchase a 3-month extension for $175.
• Payment plans run for 3 months. The first payment is due at registration. The remaining 2 equal payments will automatically be withdrawn on the same day of each of the following 2 months.
• If a card is declined for the 2nd or 3rd payment, you will immediately lose access to your training modules, and it will not be granted again until the payment is made.
• You will not receive your printed materials in the mail until all 3 payments have been made.
• I will allow Birth Boot Camp to use quotes from my training evaluations (which I may fill out anonymously) for promotional purposes online and in print. 
• Birth Boot Camp reserves the right to refuse any registrations.
